Submitting a Form 1095-C Request

In accordance with IRS regulations, United Services Group is not mailing Form 1095-C statements automatically. However, you may request a copy of your Form 1095-C at any time.

To request your statement or for questions, please use one of the following methods:

By Phone: Call 980-237-1335 Monday–Friday, 8:00 AM to 5:00 PM Eastern Standard Time

Once your request is received, a copy of your Form 1095-C will be provided within 30 days.
